Going Beyond the EHR: Supporting Care Coordination

Electronic health record (EHR) systems support physicians and other healthcare professionals in the delivery of care through effective care coordination services. Today there is growing recognition of the role of EHR systems to support individual and population-based care management in medical practices. Independent of practice size, EHR systems support physicians and care teams in multiple care management areas, including patient self-management, individual and population management, delivery system design, and clinical decision support system.

With incentives like the Medicare Shared Savings Program and the Advance Payment model recently introduced, it is an important time for physician practices to position themselves to be in the best position to qualify for incentives. The Medicare Shared Savings Program will provide financial incentives for physicians to coordinate care for patients as part of the ACO model.

Providers' shared savings will be based on quality measures such as patient outcomes and care coordination. Together, the Medicare Shared Savings Program and the Advance Payment model promote opportunities for those with the documentation to support efficient and effective care coordination of their patients to take advantage of the incentives from programs like the patient-centered medical home, account care organizations and commercial payers’ incentive programs.

Matt Adamson
, Vice President, Health Solutions
Matt Adamson has been with MEDecision since 1996 and has served as a vice president since 2007. He is currently responsible for defining the vision and strategy for the technologies that MEDecision is developing to support accountable care organizations (ACOs) and patient-centered medical home (PCMH) models. He represents MEDecision with stakeholders interested in furthering the ACO and PCMH movements in the U.S.  Overall Mr. Adamson has more than 18 years of software industry experience. He holds a degree in Management Information Systems from Eastern University of Pennsylvania.
